POSITION TITLE: GAU Care Coordinator
DESCRIPTION OF POSITION: The GAU Care Coordinator provides and coordinates mental healthcare and community referrals for clients enrolled in the GAU (General Assistance - Unemployable) pilot program, provides therapy and education, monitors symptoms and response to medication and/or psychotherapy using structured instruments, works closely with the primary care provider and the consulting psychiatrist to revise the treatment plan as necessary. The Care Coordinator is responsible for facilitating high levels of continuity of care and adjusting flexibly to changing clinic and patient needs, within general program boundaries.
MINIMUM REQUIREMENTS: Requires a Master’s degree in a mental health related field and 2 years mental health experience or a Bachelor's degree and 4 years mental health experience. Familiarity with the DSM IV manual, and an ability to conduct effective intake assessments and provide accurate diagnoses. Skilled in clinical intervention techniques which are time limited and effective. Professional oral and written communication skills. Intermediate-level keyboarding and computer user skills. Must have driver’s license, good driving record, and own automobile with insurance.
PREFERRED EXPERIENCE & SKILLS: Prefer Mental Health Professionals. Strongly prefer experience working with clients with co-occurring mental illness, chemical dependency, and medical complications. Prefer clinical mental health experience working with all adults.
STARTING PAY: $17.10 – 20.57 /hr to START depending on qualifications.
